Dear Patients,
It has been such an honor and privilege to care for you over the past 9 years. It is with sadness that I notify you of my departure. My last day at Dr Philip Penepent and Associates will be Friday, January 21, 2022.
As my children have grown it has come time to make changes for our family. I have accepted a position with Landmark Health and will be providing home care to a panel of assigned patients.
Thank you for allowing me to be a part of your life as your primary care provider. It has been such a pleasure to care for you and I value all the wonderful stories and moments that we have had. I will deeply miss the relationships that we have formed.
Please be patient with the transition process but I know that you will remain in good hands at this practice.
Again, thank you for trusting me with your care and I will truly miss you.
